Asthma Disease Management

McLaren Health Plan's asthma disease management program is a comprehensive program that begins with early identification of the asthmatic member, and through patient education and monitoring, promotes improved outcomes. The emphasis of the program is on self-management and coordination with the primary care physician (PCP).

PCPs receive notification of new members admitted to the program. Program goals include emphasis on use of peak flow meters, asthma trigger education, and achievement of regular use of inhaled anti-inflammatory agents. Ongoing individualized contacts with the member are conducted on an as needed basis to ensure understanding of treatment plans, promotion of the preventive measures and stabilization of the disease progression.

Members with a diagnosis of asthma are automatically enrolled in the Asthma Disease Management program. Members can become ineligible if they are incorrectly identified as asthmatic, are no longer an MHP member (termed/expired) or if the member chooses to opt out of the program. Managing Your Asthma: Why Your Follow-Up Visit Matters

If you just went to the hospital, emergency room, or urgent care for your asthma, please schedule a visit with your regular doctor. You should see your doctor within 30 days of leaving. Even if you feel much better, this quick visit is a vital step to stay healthy. Your doctor will check your medicines and update your Asthma Action Plan. This helps prevent another flare-up. Taking this step puts you back in control of your health. It helps you avoid another unexpected trip to the hospital so you can breathe easier every day.

  • Call right away: Do not wait to call. Doctor schedules fill up fast. Try to book your visit within a few days of coming home.
  • Bring your paperwork: Bring the papers from your hospital or urgent care visit. This helps your doctor see the care you received.
