Wellness Programs and Services

McLaren Health Plan (MHP) is committed to a proactive approach to patient management. To promote the well-being of each member, we focus on disease management and preventive health management. Early assessment of  members for chronic disease, special needs and preventive education is essential to improving outcomes, changing lifestyles, controlling cost and increasing membership satisfaction.

Medical Management supports the needs of members and the provider network. Medical Management offers support to coordinate our members’ care and to facilitate access to appropriate services through the resources of our nurse case managers. If a MHP member is identified as having a condition such as Asthma, Diabetes, Depression, or is pregnant, our case management department initiates a disease management process. Eligible members are continually identified through claims and encounter data, pharmacy reports, in-patient hospitalizations and through direct referral from the primary care physician.

In addition to member focus, the nurse case managers are available to assist our provider network with health care delivery to our members. The nurses are available for members 24 hours a day, seven days a week and work under the direction of MHP’s Chief Medical Officer. Disease management provides a broad focus for the member while addressing a wide range of needs and allowing for more control of the member’s condition.
