McLaren Health Plan's HIV management program begins with early identification of members living with HIV and members that are at risk of developing HIV. Through member education and monitoring, the program promotes improved outcomes. The emphasis of the program is on reduction of the viral load for members living with HIV and prevention of HIV for members at risk of developing HIV.
Ongoing individualized contacts with the member are conducted on an as needed basis by a nurse to ensure understanding of viral load for members living with HIV and understanding of HIV prevention strategies for those members at risk of developing HIV. Additionally, we encourage and assist our members to connect with a health care provider if needed to establish care.
Members with a diagnosis of HIV and members at risk of developing HIV are automatically enrolled in the HIV Management program. Members can become ineligible if they are incorrectly identified as living with HIV or incorrectly identified as at risk of developing HIV; are no longer an MHP member (termed/expired); or if the member chooses to opt out of the program.
