Welcome to McLaren Health Plan Healthy Michigan!

Make sure you use your McLaren Health Plan Healthy Michigan ID card when you need medical care. The name of your Primary Care Physician (PCP) is on the card. It is your responsibility to see your PCP within 60 days of becoming a McLaren Health Plan Healthy Michigan member.

At your first appointment you must have your Health Risk Assessment (HRA) completed. An HRA is required for every McLaren Health Plan Healthy Michigan member. It is a tool to help you get healthy and stay healthy. Please call McLaren Health Plan at (888) 327-0671 to get your HRA started and sent to your PCP before your first appointment.

When you complete your HRA with your PCP, your PCP will talk with you about your health needs. Your PCP will also ask you if you are ready to make any changes to improve your health. Your PCP will help you select a healthy behavior goal. As a Healthy Michigan Plan member, it is very important for you to select and achieve your healthy behavior goals. Changes are coming to Healthy Michigan Plan in January 2020

Starting, January 1, 2020, Michigan law requires some people with Medicaid health care coverage through the Healthy Michigan Plan (HMP) to work or do other activities, like job search, for at least 80 hours each month.  If you do not meet this requirement, you may lose your health care coverage.

You must report work or other activities if you:

                   
  • Are between ages 19 and 62
  • Have Medicaid health care coverage through the Healthy Michigan Plan
  • Don’t have a reason to be exempt (excused from the new requirements)
