2012 Sales Sold Group Checklist:
At McLaren Health Plan, we strive to give all of our agents and groups exceptional service throughout the selling and group installation process. We know it is important to you that the groups that choose McLaren Health Plan are service ready on their effective date.
To ensure our groups receive the exceptional service they deserve, please use the below checklist to ensure all Sold Group Information has sent to McLaren Health Plan:
- Group Agreement (5 pages and completely filled out, including all signatures)
- Plan chosen by the group (Copy of the BIB.)
- Enrollment/Waiver Forms (Must have DOB’s for everyone enrolled, PCP chosen, Signature of the EE and the group administrator.)
- Binder check (One month’s premium – Small Groups Only)
- Group Roster (3 pages and must have name printed and signed by the Group Administrator)
- Most recent Quarterly Wage Detail Report
- Prior Carrier Invoice
- Group Status Verification Form
- Delta Dental Information, including Binder Check (If applicable)
